Problem:

After launching Mediata Survey, even before reaching the splah window, or immediatly after showing it, the application immediatly quits

Solution:

There is a know issue with Mediata Survey and some anti spyware / anti virus software that won't let Mediata Survey start, believing it is a malware application because is trying to write a preferencei file in the USERDATA folder.

The workaround to this issue is to set the property "Windows 98 compatibility" when right click on the application icon, selecting "Properties".
