Issue:

All fields in the form appear except for the SUBMIT, RESET and IMAGE buttons.

How to solve:

These buttons are not required for script generation in Forms To Go. They are suppressed from displaying by default. To display them, uncheck SUBMIT, RESET and IMAGE checkboxes accordingly under:

Windows: Settings > Options > HTML Form

Mac OS X: Forms To Go menu > Preferences > HTML Form

This setting applies to loading forms.

NOTE: Some HTML editors do not assign the NAME attribute to the SUBMIT and RESET buttons. After loading a HTML form in Forms To Go, you will see -missing field name- for these buttons. The solution is to add the NAME attribute to these buttons in your HTML editor or configure Forms To Go to suppress display of these buttons.
